Job Summary
Plans and coordinates education and care of patients/persons with diabetes across a continuum in collaboration with other disciplines, community agencies and Sentara subsidiaries. Provides staff education, educates/counsels patients to promote wellness and prevention of complications in persons with diabetes. Serves as a clinical resource person for the clinical staff serving the inpatient population.Qualifications:
